

CHECKLIST OF INDICATORS FOR SCHOOL PERSONNEL TO HELP
IDENTIFY CANDIDATES FOR LEEP
If the student has or you believe has one or more of the following characteristics then we would want to discuss how LEEP could help by having you talk with one of our staff. We suggest that you discuss this first with staff only so that students are not set up for disappointment should they not meet grant eligibility that the federal government has imposed as a condition for the Jefferson Chamber/LBLN receipt of funding for this project. The project works with youth/ young adults with disabilities (16-25) so that they may enter into employment and succeed vocationally. The disability has to be one that can be medically or psychologically assessed through either existing records or LEEP project staff evaluation but need not be already identified as such in school or adult service records.
